New Patient Registration
We are currently welcoming new patients at Bilborough Medical Centre. If you live in Bilborough or Nuthall and would like to register with us, please collect and complete one of our registration forms and a health questionnaire. These are available from reception or can be downloaded from the "Registering Online" tab.
If you have registered online you still need to come into the Practice to sign your form and to provide the following:
- Medical card
- Photo identification along with proof of address (e.g. recent utility bill)
- Red books for children under 5 years of age.
All new patients over the age of 15 are offered a health check with our practice nurse or healthcare assistant; this appointment will be made when registering. If you are taking any repeat medication please bring your medication or your prescription counterfoil with you to your health check appointment so that we can review the medication whilst we are waiting for your medical records to arrive at the surgery.
We regard this as an important part of your healthcare and we would encourage all patients to make and attend this appointment. If you are unable to attend an appointment please inform the Health Centre and a new appointment will be made for you.
