New Patient Registration


We are currently accepting new NHS patients. It's not possible to register online, but you are welcome to print out the following 2 documents, fill them in and then bring them to the reception desk at the practice when you want to register. This will speed up the registration process for you and us.

When you attend the practice to register, please bring with you one piece of Photo ID (e.g. a passport, driving license) and proof of your address (e.g. recent utility bill).

Some patients will be invited for a health check appointment with our nurse or healthcare assistant when they register. This includes patients over the age of 40, those with long term health conditions and other specific conditions. This appointment will be made for you when you register. If you are taking any repeat medication please bring your medication or your prescription counterfoil with you to your health check appointment so that we can review the medication whilst we are waiting for your medical records to arrive at the surgery.
